Wintringham provides aged care services to those who are financially disadvantaged and have previously been homeless or are at risk of becoming homeless. We employ over 1000 staff and offer affordable, high quality accommodation to people aged 50 years and over. We provide a safe, flexible and caring family-style environment, which allows residents to maintain their chosen lifestyles, habits and activities and to be as independent as they choose to be.

 

Your new role

An exciting new opportunity exists for experienced and enthusiastic Case Managers to join our friendly, Management team within Housing and Homelessness Support. This role is on a Casual Basis allowing for greater flexibility in your roster. The HHS – Casual Housing Support/Case Worker will work collaboratively with Coordinators, other Case Workers and Housing Support staff, and other members of the multi-disciplinary teams to ensure a cohesive team culture is maintained. The Case Manager will also have an advocacy and linkages focus, assisting older people to engage in clinical and social supports via comprehensive assessment, emotional support, service coordination, and referral.

Your duties and responsibilities will include:

  • Support clients using techniques, principles and frameworks including Harm reduction and brief intervention, Motivational interviewing and coaching, Trauma informed practice, Conflict resolution, MARAM, Housing First
  • Work collaboratively with clients to create a meaningful life, supporting them through a staged approach to independent living, community engagement and recovery.
  •  Support clients to identify strengths, create individual case plans, and to increase skill and capacity development.
  • Facilitate and encourage the engagement and maintenance of natural supports.
  • Work with clients by linking and referring them into clinical services nd social.

  • Assist clients to maintaining their tenancy by working in partnership with Wintringham’s Tenancy Management team
  • Ensure clients obtain access to Commonwealth Home Support Program (CHSP), Home Care Packages (HCP) and National Disability Insurance Scheme (NDIS) by providing referrals and case management assistance.

  We are looking for candidates with the following:

  • Tertiary qualification(s) in Social Work, Occupational Therapy (Level 3 ) and/or related Allied Health Discipline
  • First Aid Level 2
  • Previous experience in Homelessness and or Aged Care sector
  • Current driver’s license
  •  Experience in Support Worker role in Intensive Case Management
  • Demonstrated understanding of My Aged Care and Housing and Homelessness.
  • Experience providing case coordination across a range of service providers
