Wintringham is a specialised non-religious not-for-profit welfare organisation. We are committed to providing older people who are at risk or have experienced homelessness with a range of high-quality care, housing and services. We employ more than 1,000 staff across Victoria and Tasmania.

 

Your new role

This is an exciting opportunity for a professional experienced with Intensive Case management, eager to demonstrate their exceptional leadership skills within our HHS Team. In addition to working collaboratively with the Housing and Homelessness Support (HHS) Coordinator to deliver HHS Programs, this role will identify barriers which prevent access to home care programs for marginalised older people and develop strategies to overcome these. As a point of escalation, you will manage issues and incidents on-site and ensure effective communication with key stakeholders and the Site Coordinator. This role will also identify gaps in current service provision and make recommendations for future programs which could be developed by Wintringham or the local community.

 

Your fortnightly roster will be as below:

Week 1: Monday, Tuesday , Thursday and Friday (0900 – 1700)

Week 2: Monday, Tuesday , Thursday and Friday (0900 – 1700)

 

Your responsibilities and duties will also include:

  • Provide a key leadership and supervisory role that guides and develops the program through a range of innovative and creative plans and strategies.
  • Provide formal supervision and mentoring to the Lionsville Housing Support , and the HSAP team
  • Effectively supervise HHS staff whilst managing a case load of HHS clients 
  • Respond to and manage issues and incidents on-site ensuring information is shared with the Western Co-ordinator.
  • Ensure recruitment of individuals who have the right cultural fit and ability to work in line with cultural change and facilitate retention of key talent.
  • Oversee and supervise case planning and interventions to meet the needs of people with multiple and complex needs
  • Ensure clients obtain access to Home Care Packages (HCP) and NDIS by providing referrals and case management assistance
  • Review client file documentation across program areas to ensure the timely and accurate recording of information and monitor service delivery for quality
  • Respond to client feedback and complaints in line with Wintringham policy and procedures
  • Utilise relationships within sectors to assist access into other services and systems

We are looking for candidates with the following:

  • A tertiary qualification in Social Work or equivalent.
  • Experience providing practice supervision to a dedicated team
  • Experience in a senior role working with people with complex needs
  • Demonstrated experience providing secondary consultation within a case management framework
  • Proven service delivery experience and commitment to excellence, accountability and innovation in best practice service delivery in the fields of homelessness and housing support
  • Demonstrated ability to foster a collaborative and synergistic spirit across teams, focussing on individual and team wellbeing
  • Knowledge of Commonwealth and State government policies, standards and compliance requirements
  • Knowledge of and commitment to Continuous Quality Improvement
  • Demonstrated skills in financial management, including monitoring and reporting of variances
  • First Aid Certificate.

Successful candidates will be required to comply with the Wintringham Health and Immunisation policy including having three doses of a COVID-19 vaccine as well as a 2024 Influenza Immunisation vaccination before commencing employment and must provide evidence.
