Wintringham is a Not for Profit organisation that employs over 800 staff and provides aged care services to those who are financially disadvantaged, have previously been homeless or are at risk of becoming homeless. Wintringham is an advocate for elderly people, respecting their individuality, whilst working to achieve equality and social justice.

 

Your new role

The Older Person’s Prison Program team work directly with clients, corrections staff and other support agencies to gain a greater understanding of the care needs of aged prisoners in preparation for release.  This position will assess ageing needs, undertake case planning and extensive care coordination to enable a smooth transition into aged care services post release.  

 

Your responsibilities and duties will include:

  • Support aged people to receive appropriate support and care upon release from prison services.
  • Support a person’s transition into aged care, housing, health & welfare services.
  • Undertake psychosocial assessments to inform discharge planning.
  • Perform outreach in nominated Victoria Prisons and in the community.
  • Collect, analyse, and report on ageing prison population data.
  • Liaise with relevant stakeholders including parole & prison staff, community health services, and aged care services.

We are looking for candidates with the following:

  • Tertiary qualification in Social Work, Community Services or equivalent.
  • Experience in one or more of the following sectors: Homelessness, Prison outreach, community case management or case coordination, Aged Care.
  • Ability and desire to work with clients with challenging and complex needs.
  • An understanding of trauma informed practice and harm minimisation.
  • Excellent communication skills with the ability to engage people from varying backgrounds.
  • Current drivers licence (mandatory)
  • Own reliable vehicle that is suitable for transporting clients and work purposes (mandatory)
