The exciting opportunity

• Permanent, full time role working Monday - Friday
• Manage the care provided to Support at Home clients in the Kiama and Wollongong areas
• Create, review and update holistic care plans that support independence and achievement of goals
• Manage the referral process to allied health professionals and brokered services
• Guide and empower Support Workers to deliver care that meets clients’ needs and wants
• Educate clients on budget allowances and monitor spending
• Complete comprehensive documentation, maintaining accurate and timely records

About you

• Experience as a Care Partner or Home Care Coordinator, with an understanding of Support at Home
• Superb relationship building and customer service skills with a passion for supporting the elderly to live independently and achieve goals
• Flexible, open minded, resilient, results oriented, demonstrates initiative and able to work autonomously
• Must have a vehicle, unrestricted driver's license and full working rights in Australia
• NDIS Worker Screening Check and flu vaccination, or willingness to obtain
