About the Opportunity

 

This newly created part time role reports directly to the Disability Services Manager and works closely with the NDIS Quality and Compliance Coordinator.

You will play a crucial role in building strong rapport and working relationships with a range of stakeholders for our NDIS participants living in our Residential Aged Care homes, whilst ensuring you support the Residential Aged Care staff to navigate NDIS processes and understand supports in our participants NDIS plan.

You will have an excellent focus and understanding of the regulatory responsibilities and interface of both the Aged Care and NDIS systems.

With your communication skills, planning, organisational and time management skills you will be a key contact person for our NDIS participants plans and ensure alignment to Hall and Prior values and policies.

 

What does success look like?

 

  • Manage NDIS on-boarding for new referrals in your caseload.
  • Conduct home visits and participate in stakeholder/NDIS/MDT meetings, for participants.
  • Ensure all participant information is accurately documented and updated in their NDIS profiles. 
  • Maintain diligence and attention to detail in all work processes.
  • Identify and manage NDIS restrictive practice processes / incident reporting/feedback reporting in consultation with Disability Services Manager and NDIS Quality and Compliance Coordinator.
  • Inform NDIS and Aged Care Teams of any participant-related changes.
  • Use data, metrics and external market insights to drive and influence decisions.

We are looking for someone with the following qualities:

 

  • Experienced in managing an ongoing case load of participants.
  • Outstanding communicator with a proven track record of building strong relationships with participants, stakeholders and team members.
  • Ability to work autonomously as well as being a team player.
  • Previous experience with NDIS and Aged Care with an understanding of both sectors and funding.
  • Proven ability in writing, and developing reports.
  • Proficient in electronic document management and strong computer skills.
  • Effective problem solving skills and daily operational adaptability.
  • Demonstrated ability to prioritise workloads and organise tasks.
