Position Overview 

The Aged Care Clinical Coordinator plays a crucial role in overseeing clinical supports and ensuring clinical governance requirements are met within our Aged Care service. You will collaborate with a small Aged Care team to plan and deliver tailored services for individuals, ensuring their needs and preferences are prioritised. 

The position requires strong clinical expertise, excellent communication skills, the ability to foster effective relationships with customers, and their families, and other healthcare professionals. You will be responsible for developing care plans, coordinating services, and monitoring compliance with regulatory standards. 

This part-time, hybrid position (30 hours per week) can be worked flexibly across Monday to Friday as negotiated. Working hours can also be flexible to suit school hours.

Candidate Profile 

We are looking for a skilled and compassionate Aged Care Clinical Coordinator who is dedicated to enhancing the lives of older people. The ideal candidate will have a strong clinical background, a commitment to person-centred care, and the ability to navigate the complexities of the aged care sector. 

  • Bachelor's degree in nursing with current APHRA registration 
  • Comprehensive understanding of the Aged Care Act 2024 and related legislation 
  • Experience in clinical assessment and community aged care 
  • Proficiency in care planning and referral processes for aged care services 
  • Skilled in using digital care systems and technology to support efficient and accurate case management
  • Strong problem-solving and decision-making skills 
  • Commitment to quality assurance and continuous improvement 

As an Aged Care Clinical Coordinator, you will demonstrate a deep understanding of the values and principles that guide our service delivery. Your ability to advocate for customers, maintain accurate documentation, and ensure compliance with safety regulations will be essential to your success in this role.
