Position Overview

The Care Coordinator is tasked with the important role of coordinating our Aged Care Service. You will ensure service continuity, compliance with legislative standards, and foster a culture of continuous improvement. You will deliver personalized, person-centred care to our customers, ensuring their needs and preferences are respected and prioritised.

This position requires effective communication and collaboration with customers, their families, and a network of health professionals to ensure the highest quality of care. You will be responsible for ongoing assessment and care planning, compliance monitoring, rostering, management of support staff, and fostering a safe working environment.

Candidate Profile

We are seeking a dynamic and compassionate individual with a strong background in aged care coordination to join our small dedicated Aged Care team. The ideal candidate will possess the following qualifications and experience:

  • Certificate IV in Aged Care or Disability, or equivalent experience
  • Demonstrated experience in delivering community services, with a focus on person-centred care
  • Leadership and supervisory skills
  • Excellent communication and interpersonal skills, with the ability to engage with customers and stakeholders effectively.
  • Proficiency in digital systems and documentation processes

Successful candidates will demonstrate a commitment to promoting dignity, choice, and inclusion for customers while adhering to best practices in aged care service delivery.
