Your new role

Reporting to the Senior Social Worker, the Social Worker is responsible for delivering specialised social work support services and targeted case management interventions to older people who have experienced homelessness or are at risk of homelessness. This role operates within the Support at Home model, providing billable services including complex service navigation, crisis intervention, advocacy, and emotional support, with the aim of supporting older people to remain safe and independent at home.  Additionally, this role also provides CHSP -Social Work services to clients.

We are looking for candidates with the following

  • Bachelor of Social Work or equivalent.  Eligibility for membership with the Australian Association of Social Workers (AASW)
  • Demonstrated experience working in provision of services to people who are ageing, and/or have experienced homelessness, marginalisation, and disadvantage.
  • Familiarity with aged care laws, elder rights, and ethical decision-making frameworks.
  • Demonstrated knowledge of aged care systems, homelessness, CHSP, NDIS, and legal frameworks like VCAT, Residential Tenancies Act, Coroners Court, and the Aged Care Act.
  • Commitment to the organisation and its philosophy of social justice.
  • Highly developed communications skills and the ability to engage with people from varying backgrounds and networks.
  • Experience in coordinating and delivering care and support services to participants.

  • Competitive pay ranging from $47.51 to $51.13 (BSc degree is required as per PD) per hour, depending on experience, plus superannuation.
