Your new role

The Care Partner - Home Support is responsible for delivering and coordinating person-centred care management and service coordination that supports older people to maintain their independence, wellbeing and quality of life while living in the community. These services are federally funded through the Support at Home program. Reporting to the Senior Care Partner this is a full-time permanent role, located in Ballarat or Bendigo.

We are looking for candidates with the following

  • Qualification in Community Services, Aged Care, Disability, Allied Health, Nursing, Social Work, or a related field. First Aid certificate – Level 2 and Valid Victorian drivers licence.
  • Demonstrated experience in care management, case management or service coordination within aged care, health, disability, homelessness, or community services, including supporting clients with diverse and complex needs.
  • Proven ability to undertake client assessments, develop care plans and coordinate services that support participants ’goals, wellbeing, independence, and quality of life.
  • Strong understanding of person-centred practice, client rights, and dignity of risk, with the ability to support informed decision-making and client choice.
  • Demonstrated ability to identify, assess, and respond to participant risks and changing needs, including coordinating referrals, reassessments, and appropriate interventions.
  • Experience managing participant budgets, package utilisation, service agreements, and care management activities in accordance with funding requirements and organisational expectations.

What we offer

  • This is a full-time permanent role, primarily based from our Ballarat office. Regular time in Ballarat will be required, however a Bendigo base may be considered for the right applicant.
  • Competitive salary: $40.52 - $50.19/hour plus super (depending on experience and qualifications). Access to exclusive not-for-profit salary packaging: salary package your personal expenses up to the amount of $15,900.
