Your new role

The Support Partner - Home Support is responsible for providing service coordination and technical and administrative support to the care management team, enabling the effective delivery of participant care plans and Home Support services. The role coordinates service delivery, supports participant onboarding and transitions, maintains participant records and documentation, and facilitates communication between participants, service providers and internal teams. Reporting to the Senior Care Partner this is a full-time permanent role, located in Braeside.

We are looking for candidates with the following

  • Qualification in Business, Administration, Aged Care, Disability, Allied Health or equivalent experience. A valid Victorian drivers licence and First Aid certificate – Level 2 (or willingness to obtain one).
  • Demonstrated experience in administration, service coordination, customer service or participant support within aged care, health, disability, community services or a related environment.
  • Strong administrative and organisational skills, with the ability to manage multiple priorities, coordinate activities, maintain accurate records and meet competing deadlines.
  • Proficiency in participant management systems, databases and Microsoft Office applications, with a high level of accuracy in data entry, record management and reporting.
  • Demonstrated ability to follow established processes, maintain confidentiality and exercise sound judgement, including the identification and escalation of risks, concerns and service issues.
  • Well developed communication and interpersonal skills, with the ability to build positive and effective relationships with participants, families, carers, service providers and colleagues.

  • This is a full-time permanent role, based in our Braeside office with the ability to work some days from home.
  • Competitive salary: $40.67 - $ 44.91/hour plus super (depending on experience and qualifications). Access to exclusive not-for-profit salary packaging: salary package your personal expenses up to the amount of $15,900.
