  • Permanent, full-time role supporting clients across Townsville.
  • Starting salary: $76,187.23 + 11.5% Super + tax-free salary packaging up to $18,550 per year.
  • Company pool car available or reimbursement of $0.96/km when using your own vehicle.
  • Hybrid role – work from home and in the community.

At integratedliving, we’re a team of 1,100+ dedicated professionals transforming the lives of elderly Australians in rural and remote communities. Our Case Managers play a key role in this mission, helping clients access tailored services and supporting their independence.

 

Your Role

As a Case Manager, you will:

  • Work closely with clients, families, and healthcare professionals to develop personalised care plans.
  • Empower clients to make informed choices about their Home Care Packages.
  • Ensure services are optimised to enhance independence and wellbeing.
  • Collaborate with internal teams and external partners to deliver seamless support.

About You

You're passionate about health and wellbeing, skilled at building relationships, and eager to make a difference in people’s lives.

You’ll bring:

  • A Certificate III or higher in Case Management, Aged Care, Community Services, Disability Services, or a related field.
  • Experience in case management across Aged Care, Disability, or Youth services.
  • Strong communication skills with a commitment to person-centred outcomes.
  • A valid driver’s license and the ability to work both independently and as part of a team.
