Clinical Case Manager - Empowering Older Adults to Thrive.

As a Clinical Case Manager, you will play a pivotal role in our 8-week Short Term Restorative Care Program.  The role is to provide care management and coordination, not care delivery.  Administration tasks are no match for your meticulous attention to detail and knack for keeping things organised

 

 Role Details 

 

  • Full Time Contract - Monday to Friday
  • Partner with clients to achieve wellness outcomes in Townsville
  • Competitive base rate plus superannuation with eligibility to salary packaging (up to $18.5K tax free)
  • Ability to work from home with some travelling required for face-to-face assessments & service promotions with clients, as per business requirements 
  • Travel reimbursement payment for use of own private vehicle.

You will work collaboratively to improve the quality of clients care through facilitation the efficient use of resources thereby enhancing quality, cost-effective outcomes. 

 

At integratedliving, our Clinical Case Managers work collaboratively with other health professionals to assess, plan and implement restorative or reablement activities for our clients.

 

 The role focuses on prioritizing and personalizing health management through health literacy, coaching, education, and assistive technologies to empower clients and their families in achieving optimal health and well-being.

 

  • Visit clients in their homes to assess and coordinate Short-Term Restorative Care in Townsville
  • Work remotely with supervision and within Aged Care Standards
  • Partner with clients to create personalized care plans and support their health goals throughout the 8-week program
  • Educate clients and families on wellness and chronic disease management
  • Facilitate group workshops and coordinate with a multi-disciplinary team for holistic care
  • Promote the program and build relationships with local health professionals

 

What you need to be considered for the role?

 

  • Degree in nursing or allied health with AHPRA registration
  • 3+ years of clinical or health management experience in Nursing or Allied Health within a health or community aged care setting  
  • Demonstrated understanding of community aged care guidelines and evidenced based care
  • Ability to conduct Clinical Case Management and Health assessments 
  • Strong clinical, assessment, problem-solving, and reporting skills with the ability to work both independently and as part of a team
  • Strong time management and IT skills, with experience in virtual health services
  • Current driver’s license and ability to travel within the region regularly
