  • Permanent Full-Time Position

  • Monday to Friday, 38 hours/week – No weekends!

  • Flexibility to work with clients in person in the Hunter region and occasionally work from our Wellness Centres

integratedliving is seeking a compassionate and skilled Social Worker to join our Allied Health team in the Hunter region. This is a meaningful opportunity to support older Australians with personalized care, both face-to-face and through Telehealth. You will conduct assessments, provide clinical care, and advocate for clients with a diverse range of needs. Your role will be pivotal in delivering support through programs like Home Care Packages (HCP), Commonwealth Home Support Program (CHSP), and Short-Term Restorative Care (STRC).

 

Additionally, you’ll have the chance to contribute to cross-functional projects and, if interested, lead group discussions. Join a multidisciplinary team of skilled professionals who are passionate about delivering the best outcomes for clients.

  • Conduct comprehensive health assessments to assess client needs, create care plans, and implement targeted actions

What You’ll Need to Succeed:

 

  • Qualifications: Bachelor’s degree (or higher) in Social Work with AASW membership and accreditation

  • Compliance: Satisfactory police check, valid First Aid and CPR certifications

  • Mobility: Current driver’s license and willingness to travel for client visits

  • Skills: Strong communication and interpersonal skills, proficient with client management software

  • Experience: Ability to work effectively in a multidisciplinary team, with sound clinical knowledge and experience handling complex cases
