Are you a Registered Nurse or Allied Health Professional looking to take the next exciting step in your career? We are currently inviting internal expressions of interest for a Wellness for Independence (WFI) Lead to join our Health Services team in a 12-month fixed-term contract, covering a maternity leave.
Position Overview
- Part-time | 12-month Maximum Term Contract
- Workdays: Monday and Wednesday (8:30am – 5:00pm) + 4 additional hours
- Location: 100% Work from Home

About the Role
As a WFI Lead, you’ll play a key role in promoting wellbeing and independence for older Australians by leading the delivery, improvement, and education of our award-winning Wellness for Independence® programs. You’ll coach and support Health Services team members, engage with clients and carers, and foster continuous improvement across our service delivery.
Working collaboratively within a multidisciplinary team, you’ll contribute clinical expertise, enhance client outcomes, and ensure our programs continue to reflect best practice in community and virtual care.
Key Responsibilities

Conduct initial and ongoing clinical health assessments and develop individualised care plans

Promote and deliver WFI programs virtually and in-person

Provide coaching, training, and education to Health Services staff and partners

Lead health promotion activities and support client self-management

Assist clients across various funding streams (HCP, CHSP, etc.) with direct clinical care

Contribute to service development, policy updates, and continuous improvement initiatives

Collaborate with internal stakeholders to enhance care coordination and client outcomes
What You’ll Bring

Professional Registration & Experience
-
Current registration as an RN (AHPRA) or Allied Health Professional (AHPRA, AASW, or DA) with 4+ years of relevant clinical experience.

Qualifications (Desirable)
-
Graduate Certificate in Chronic Disease, Aged Care, Community, or Primary Health, and/or Certificate IV in Training & Assessment.

Health Program & Coaching Expertise
-
Experience delivering virtual wellness programs, health coaching, and engaging with multiple stakeholders.

Strong Communication & Training Skills
-
Proven ability to present, train, and communicate effectively with diverse teams and clients.

Compliance & Tech Skills
-
Proficient in Microsoft Office, electronic documentation systems, and holds current First Aid, CPR, and a satisfactory Police Check.
