Please download and read the IPC Health's Code of Conduct before going any further. IPC Health takes the conduct of our team very seriously and you will be asked to confirm your acknowledgment and understanding of this code before you can submit your onboarding information.
Please complete the following information and click next. On the second page you will be able to attach certified copies of your right to work documentation, qualifications and licences (if not already collected in the interview process).
Such confirmation can be provided in any one of the following options:
- A clear scan or photo of your current (or expired within 2 years) Australian or New Zealand Passport photo page; OR
- A clear scan or photo of your Certificate of evidence of resident status; OR
- A clear scan or photo of your valid Visa with work rights and passport; OR
- A clear scan or photo of your Australian Citizenship Certificate; OR
- A clear scan or photo of your Full Australian Birth Certificate (if born before 20 August 1986) and a form of photo ID; OR
- A clear scan or photo of your Full Australian Birth Certificate (if born on or after 20 August 1986), and a form of photo ID and evidence that at least one parent was an Australian citizen or permanent resident at the time of your birth

If a Working With Children Check is required as part of your position, please ensure a copy of your check is uploaded in the attachments section. Please also click here to update your employer status to IPC Health - PO BOX 171, Deer Park VIC 3023. This will enable IPC Health to be updated with any changes to your status during the course of your employment.
