The role:
The Clinical Care Coordinator provides clinical governance, oversight and operational management for the business in the areas of NDIS and Home Care Packages. The role involves overseeing care plans for complex clients and ensuring our community support professionals are trained and assessed in complex skill management. This role is responsible for ensuring excellence in service delivery and compliance with all relevant professional standards, industry standards and legislative requirements. This role will provide effective leadership, supervision, and clinical support to staff to ensure the smooth running of the team.
The Clinical Care Coordinator will ensure care is provided in accordance with the relevant Professional and Industry Standards, and all legislative requirements.
Duties and responsibilities include:
- Providing clinical governance and continuous improvement to the business.
- Providing training, coaching, skills assessment and guidance.
- Ensure overall delivery of quality services, compliant systems and related governance.
- Provide effective leadership within the Service Delivery Team.
- Providing oversight of customer care plans, medication administration and complex clinical care.
- Overseeing incident management and risk management.
- Providing assessment and oversight of customers living with dementia or cognitive impairment
- Provide falls risk assessments and falls prevention management
- A demonstrated commitment to best practice.
What you will need:
- Current National registration as a Registered Nurse.
- At least three years’ experience in Aged Care in community setting.
- Evidence of continued professional development in accordance with the National Registration Board’s standards.
- Current and clear Criminal Record Check.
- Current Working with Children Check (where required in accordance with state based legislation).
- Fully immunized against COVID-19.
- Current Driver’s License.
- Current First Aid certificate.
- Highly proficient computer skills including Microsoft Office e.g. Excel, Word, Outlook.
- Experience in the supervision of office staff and community care workers.
- Demonstrated experience in assessing, care planning and evaluating complex care needs.
- Solid understanding of Aged Care Quality Standards and Principles and other aged-care and disability related legislation.
- Knowledge of continuous quality improvement principle's.
- Proven leadership skills and an ability to work collaboratively with stakeholders e.g. Medical teams, Allied Health, Social workers and General practitioner’s.
- Solid understanding of Wound care management in the community.
