We are currently looking for community based Registered Nurses to join our Perth team. The role:
This role involves travelling between customer’s homes and providing individual and personalised support to customers in accordance with their support plans and/or medical practitioner’s directions.
Duties and responsibilities may include:
- Working with or under the guidance of medical practitioners and Allied Health professionals
- Coordinating the assessment, planning and intervention of continence management for each customer
- Medication Administration and Complex Clinical Care e. palliative care, bowel care/suppositories, catheter care, stoma care, oxygen and more
- Monitoring and communicating on the customer’s wellbeing and reporting on progress of customers
- Ensuring that all services are provided in a caring and respectful way in accordance with Just Better Care’s policy and procedures, legislative requirements and program specific guidelines (i.e. DVA Community Nursing)
- Providing training, coaching and guidance to Community Support Professionals, Enrolled Nurses and Support Staff/Case Managers as required
- Conducting skills assessments of Community Support Professionals performing complex care services as required
- Demonstrating a commitment to best practice and evidence of continued professional development
- Participating in staff meetings and attending training opportunities when required
What you will need:
- Current Registered Nurse registration with Australian Health Practitioner Regulation Agency (AHPRA)
- Evidence of continued professional development in accordance with the AHPRA standards
- Current Criminal Record Check and Working with Children Check
- Current CPR / First Aid Certificate
- Current Australian Driver’s Licence
- Reliable, registered and insured vehicle (comprehensive insurance is preferred)
- Smartphone with internet access
- Excellent leadership, written and verbal communication skills
- Computer literacy
- Experience in the supervision of staff members
- Knowledge of continuous quality improvement principles
- Excellent people, problem solving, and conflict management skills
- Ability to work as part of a team and maintain good working relationships
Desirable:
- Previous experience in the community care or continence assessment/care or health/aged care sector
- Understanding of DVA Community Nursing guidelines or previous community nursing experience
- Cert IV in Workplace Training & Assessment
