Duties and responsibilities include:

  • Travelling between customer’s homes and providing individual and personalised support to customers in accordance with their support plans and/or medical practitioner’s directions.

  • Working under the guidance and supervision of medical practitioners, working with allied health professionals, monitoring and communicating the customer’s condition, and other health and wellness related activities.

  • Demonstrating a commitment to best practise, ensuring that all services are provided in a caring and respectful way in accordance with Just Better Care’s policy and procedures, and legislative requirements.

  • Medication Administration and Complex Clinical Care i.e. palliative care, bowel care/suppositories, catheter care, stoma care, oxygen and more

  • Monitoring and communicating on the customer’s wellbeing and reporting on progress of customers

  • Ensuring that all services are provided in a caring and respectful way in accordance with Just Better Care’s policy and procedures, legislative requirements and program specific guidelines (i.e. DVA Community Nursing)

  • Providing training, coaching and guidance to Community Support Professionals and Support Staff/Case Managers as required

  • Conducting skills assessments of Community Support Professionals performing complex care services as required

  • Demonstrating a commitment to best practice and evidence of continued professional development

  • Participating in staff meetings and attending training opportunities when required

 

About you:

  • Caring, compassionate & friendly

  • Demonstrated high level verbal and written communication skills.

  • Reliable & dependable

  • Proven relationship management with all internal and external stakeholders, as well as good interpersonal, negotiation and problem solving skills

  • Ability to work as part of a team

  • Comfortable using technology (i.e. mobile devices) for accessing rosters & recording customer assessments & documenting feedback

 

What you will need:

  • Current Enrolled Nurse registration with Australian Health Practitioner Regulation Agency (AHPRA)

  • Evidence of continued professional development

  • NDIS Worker Screening Clearance / Criminal Record Check

  • Current CPR / First Aid Certificate

  • Current Australian Driver’s Licence

  • Reliable, registered and insured vehicle (comprehensive insurance is preferred)

  • Previous experience in the community care or health/aged care sector (preferred)

  • Proof of of Vaccination (where relevant) i.e. COVID-19 vaccination certificate
