Job Purpose:

The SIL (Supported Independent Living) Facilitator performs an integral role in ensuring the efficient running of the SIL House by co-ordinating customer, staff and Just Better Care services.

Main Duties & Principal Accountabilities:

  • Provide guidance, education and direction to staff in relation to tasks that require completion during services.
  • Provide all supports as required: Personal Care, Social Support, Transport, Assist/Administer Medications, Nursing Cares (as required), Domestic Assistance, overall monitoring their well-being and reporting on progress.
  • Oversee general maintenance of house hold facilities.
  • Ensure compliance with NDIS and relevant regulations applicable to SIL house.
  • Understand customer service needs or preferences, ensuring all relevant information is accessible for team members and service providers.
  • Identify and participate in personal professional development.
  • Ensure compliance with all Just Better Care policies and procedures, statutory and contractual requirements in relation to management of service delivery.
  • Manage communication with key stakeholders, internally and externally.
  • Apply and uphold principles of equity and anti-‐discrimination in the workplace and adhere to organisational and legislative Health, Safety and Environment requirements.
  • Any other duties as required to meet the ongoing needs of the organization.

Selection Criteria:
Essential:

  • Qualifications in Nursing (Diploma or Bachelors)
  • Previous experience supervising, supporting and training staff.
  • Experience maintaining computerised customer information systems.
  • Administrative and organisational skills.
  • Written and verbal communication skills.
  • Demonstrated ability to contribute positively to the team.
  • Current and clear Criminal Record Check.
  • Current Working with Children Check (where required in accordance with state based legislation).
  • Current Drivers Licence.
  • Current NDIS Workers Screening Clearance.
