We are currently looking for an experienced Community based Registered Nurse/Coordinator to join our Townsville team. The role:

RN/Coordinator perform an integral role in ensuring individualised services are developed and delivered to customers. RN/Coordinators are responsible for the development, maintenance and review of individualised support plans in consultation with customer, their family or guardians and the key health professionals, ensuring efficient and effective delivery of professional nursing care and support to customers, respecting customer’s rights, and in consultation and cooperation with the customer, their representatives, their medical practitioner(s), and other health professionals.

This is a permanent part-time position with flexible hours starting from 20-25 hours per week with the potential to grow into 37.5 hours per week. The role involves travelling between customer’s homes and providing individual and personalised support to customers in accordance with their support plans and/or medical practitioner’s directions. 

This role involves the providing of effective leadership, supervision, and clinical support to staff where necessary, ensuring care is provided in accordance with the relevant Professional and Industry Standards, and all legislative requirements.

Duties and responsibilities may include:

  • Undertake comprehensive customer assessment and planning by assessing, monitoring, reassessing and reviewing the needs of the customer.

  • Ensure appropriate information is provided and explained to the customer at all times regarding the service agreement, including the customer handbook.

  • Where applicable, develop strategies which will address the identified needs of the customer in consultation with their representative/s where appropriate, in accordance with funding guidelines.

  • Provide financial and case management services to customers, and representatives where required.

  • Provides and delivers individual and personalised support to clients in accordance with their care plans and/or medical practitioner’s directions, responsibilities may include working under the guidance and supervision of medical practitioners, working with allied health professionals, monitoring and communicating the customer’s condition, and other health and wellness related activities.

  • Demonstrate a commitment to best practise, ensuring that all services are provided in a caring and respectful way in accordance with Just Better Care’s policy and procedures, and legislative requirements.

  • Ensure compliance with all Just Better Care policies and procedures, statutory and contractual requirements in relation to management of service delivery.

  • Manage communication with key stakeholders, internally and externally. 

  • Perform nursing services in a manner that complies with all relevant legislative, common law, regulatory requirements, and professional standards affecting nursing practise.

  • Possess, apply, and continually develop contemporary nursing knowledge to effectively support customers when developing and delivering planned care to meet their needs. 

  • Monitoring and communicating on the customer’s wellbeing and reporting on progress of customers.

  • Providing training, coaching and guidance to Community Support Professionals, Enrolled Nurses and Support Staff/Case Managers as required.

  • Conducting skills assessments of Community Support Professionals performing complex care services as required.

  • Demonstrating a commitment to best practice and evidence of continued professional development.

  • Participating in staff meetings and attending training opportunities when required.

What you will need:

  • Current CPR / First Aid Certificate

  • Current and satisfactory Federal Police Check, NDIS Worker's Screening Card & Working with Children Check Blue Card 

  • Current Australian Driver’s Licence

  • Proof of Working rights in Australia (e.g. Passport/Birth certificate)

  • Reliable, registered and insured vehicle (comprehensive insurance is preferred)

  • Smartphone with internet access

  • Registered AHPRA

  • COVID-19 Vaccination Certificate 

Desirable: 

  • Previous customer service coordination experience in the health/aged care, non for profit, community services sector. 

  • Previous experience in the community care or health/aged care sector.

  • Experience in the application of a Quality Improvement Program. 

  • Reliable motor vehicle (including valid registration and comprehensive third party insurance).

  • Certificate IV in Workplace Training & Assessment.
