Just Better Care Hawkesbury Nepean & Blue Mountains provide home and community care support services to NDIS Participants living in their own home and to CDC Home Care Packaged Consumers. 

We are looking for dedicated Registered Nurses/Package Manager to provide the efficient and effective delivery of professional nursing care and package management to customers. Our Registered Nurses provide effective leadership, supervision, and clinical support to our customers and staff where necessary. This is an office based role at our office in Richmond NSW, and will involve the provision of: 

  • Clinical oversight to maintain compliance of our customer documentation, reporting and general operations 
  • Package Management to our CDC Home Care Package Consumers and NDIS Participants
  • Train/educate, support and assess the skills of our Community Support Professionals 
  • Clinical, post hospital and general home support services to customers in their homes as required

Duties and responsibilities include:

  • Work along side a team of Package Managers and Registered Nurses to ensure comprehensive and individualised support is provided to each customer, in line with funding guidelines
  • Providing clinical oversight for customers in conjunction with advice from their medical practitioners and allied health professionals to monitor and communicate the customer’s condition, and other health and wellness related activities.

  • Oversee and complete incidents and hazards, including those of a clinical nature. 

  • Complete and maintain accurate health care plans for customers.
  • Ensuring individualised services are developed and delivered to customers. 
  • Travelling to customer’s homes and provide individual and personalised support to customers in accordance with their support plans and/or medical practitioner’s directions. Services requirements will vary, but may include Medication Administration and Complex Clinical Care i.e. palliative care, bowel care/suppositories, catheter care, stoma care, oxygen and more.

  • Demonstrating a commitment to best practice, ensuring that all services are provided in a caring and respectful way in accordance with Just Better Care’s policy and procedures, and legislative requirements.

  • Monitoring and communicating on the customer’s wellbeing and reporting on progress of customers.

  • Ensuring that all services are provided in a caring and respectful way in accordance with Just Better Care’s policy and procedures, legislative requirements and program specific guidelines.

  • Providing training, coaching and guidance to Community Support Professionals, Enrolled Nurses and Support Staff/Case Managers as required.

  • Conducting training and skill assessments of Community Support Professionals performing complex care services as required.

  • Demonstrating a commitment to best practice and evidence of continued professional development.

  • Participating in staff meetings and attending training opportunities when required.

  • Reliable & dependable

  • Proven relationship management with all internal and external stakeholders, as well as good interpersonal, negotiation and problem solving skills

  • Ability to work as part of a team

  • Experience in home and community care / case management (preferred but not required)

 

What you will need:

  • Current Registered Nurse registration with Australian Health Practitioner Regulation Agency (AHPRA)

  • Evidence of continued professional development

  • NDIS Worker Screening Clearance / Criminal Record Check

  • Current CPR / First Aid Certificate

  • Current Australian Driver’s Licence

  • Proof of Vaccination (where relevant) i.e. COVID-19 vaccination certificate
