About the role:
We are looking for an experienced full time House Facilitator, who is committed to supporting people with disabilities to live their life to the fullest in a supported independent living home.
The role is to oversee daily operations of residential setting shred home. You will lead and oversee a team of Support Workers, engage with family members, Allied Health, stakeholders, along with working directly with participants in ensuring our participants achieve their goals.
Main Duties & Principal Accountabilities:
- Provide guidance, education and direction to staff in relation to tasks that require completion during services.
- Provide all supports as required: Personal Care, Social Support, Transport, Assist/Administer Medications, Nursing Cares (as required), Domestic Assistance, overall monitoring their well-being and reporting on progress.
- Oversee general maintenance of house hold facilities.
- Ensure compliance with NDIS and relevant regulations applicable to SIL house.
- Understand customer service needs or preferences, ensuring all relevant information is accessible for team members and service providers.
- Identify and participate in personal professional development.
- Ensure compliance with all Just Better Care policies and procedures, statutory and contractual requirements in relation to management of service delivery.
- Manage communication with key stakeholders, internally and externally.
- Apply and uphold principles of equity and anti-âdiscrimination in the workplace and adhere to organisational and legislative Health, Safety and Environment requirements.
- Any other duties as required to meet the ongoing needs of the organization.
Selection Criteria:
Essential:
- Cert III in Individual Support
- Qualifications in Nursing (Diploma or Bachelors)
- Previous experience supervising, supporting and training staff.
- Experience maintaining computerised customer information systems.
- Administrative and organisational skills.
- Written and verbal communication skills.
- Demonstrated ability to contribute positively to the team.
- Current and clear Criminal Record Check.
- Current Working with Children Check (where required in accordance with state based legislation).
- Current Drivers Licence.
- Current NDIS Workers Screening Clearance.
