Clinical Care Partner - Full-Time Role (Based in Frenchs Forest)
We are searching for a Clinical Care Partner to join our growing team. Work alongside participants and their families and with their key health professionals to support the delivery of safe, high quality in-home care.

In this role you work with new and existing participants across the Northern Beaches and Northern Suburbs. You will be responsible for conducting thorough nursing assessments, developing, monitoring, and reviewing their individual care plans, and overseeing associated health care plans.
About the Role:
Clinical Care Partners play a vital role in developing and delivering individualised services based on each participant’s assessed clinical needs In collaboration with participants, their families or guardians and key health professionals, Clinical Care Partners ensure that care plans are comprehensive and tailored to meet each participant’s specific needs.
Our Clinical Care Partner provide leadership to non-clinical staff You will also maintain clinical oversight to ensure participants receive the highest standards of safe, high-quality care. You will also monitor and respond to new and emerging clinical risks, initiating reassessments as necessary to adapt to the evolving needs of the participants.
This role requires a strong commitment to care management and planning, ensuring participants receive the right care, at the right time, and in line with their goals. As an advocate for diversity and inclusion, you will also provide culturally appropriate care, while being healing-informed and trauma-aware.
Key Responsibilities:
- Conducting comprehensive clinical assessments and completing documentation in line with relevant standards.
- Developing detailed participant care plans by assessing clinical needs, goals, preferences, and existing supports.
- Coordinating services with multi-disciplinary teams to ensure seamless care delivery.
- Educating participants about their health care plans, rights, and service agreements.
- Regularly reviewing and adjusting care plans to meet changing needs.
- Maintaining oversight of participant budgets, ensure funding is used appropriately. Maintaining accurate documentation, comply with standards, and handle mandatory reporting.
- Overseeing the care plans and service coordination, ensuring they are executed safely and effectively.
- Providing leadership to non-clinical Care Partners and providing guidance on identifying and managing clinical risks.
- Providing training for Support Workers, ensuring they have the clinical skills to support participants.
- Developing strategies in consultation with participants and supporters to address their needs in accordance with funding guidelines.

About You:
Essential Qualifications:
- Current Registered Nurse registration with AHPRA.
- NDIS Worker Screening Clearance / Criminal Record Check.
- Current Working with Children
- Valid Australian Driver’s License and reliable vehicle (comprehensive insurance preferred). Just Better Care provides access to a company car for participant visits.
- Previous experience in case management or a Care Partner role.
- Strong knowledge of aged care, NDIS, and the healthcare system.
- Experience working in a multi-disciplinary team and maintaining trusted relationships.
- Experience supervising and training staff.
- Ability to work flexibly and coordinate services in line with program guidelines.
- Knowledge of continuous Quality Improvement Principles.
- Excellent problem-solving, communication, and conflict resolution skills.
- High level of administrative and organizational abilities, including experience maintaining computerised participant information systems.
