Duties and responsibilities include:
The Clinical Care Partner supports aged care and NDIS participants through assessment, planning, coordination, and ongoing review of care. They ensure safe, high-quality, person-centred services that reflect everyone’s clinical, social, emotional, and cultural needs
- Complete holistic assessments and develop care plans tailored to each participant.
- Coordinate services and support teams to deliver care aligned with participant goals.
- Monitor participant wellbeing and respond to changes in care needs or risks.
- Provide guidance and support to care staff, including ensuring training in individual care requirements.
- Engage participants in care planning, education, advocacy, and informed decision-making.
- Work collaboratively with families, carers, health professionals, and community networks.
- Maintain accurate documentation and meet program and compliance requirements.
About you:
- Caring, compassionate & friendly.
- Demonstrated high level verbal and written communication skills, including familiarity with, and confidence in using, the Microsoft Suite.
- Proven relationship management with all internal and external stakeholders, as well as good interpersonal, negotiation and problem-solving skills.
- Ability to work as part of a team.
- Part-Time roles will be considered.
What you will need:
- Qualified health or allied health professional (e.g. Social Worker, Registered Nurse, Occupational Therapist).
- Experience in care planning, case management or service coordination.
- Strong understanding of aged care, NDIS, and community health systems.
- Excellent communication, organisational and problem-solving skills.
- NDIS Worker Screening / Police Check, valid Driver’s Licence, and access to vehicle.
- Smartphone with internet access and legal right to work in Australia.
