Home Care Package Care Partner – Full Time
Location: Birtinya, Sunshine Coast
Monday to Friday, 8:30am – 4:30pm
Optional 4-day work week – enjoy a long weekend every second week

Your Responsibilities
- Conduct in-home assessments and develop individualised care plans
- Provide ongoing case management and monitor client wellbeing
- Coordinate allied health referrals, equipment orders, and follow-ups
- Manage client budgets and ensure compliance with funding guidelines
- Document goals, outcomes, and service evidence in CRM systems
- Collaborate with health professionals and regulatory bodies (e.g. ACAT, MAC, PRODA)
- Support staff training, coaching, and complex care assessments
- Liaise with clients, families, and support workers to coordinate services
- Contribute to audits, quality reviews, and continuous improvement
- (Clinical Care Partner only) Provide occasional wound care
What You’ll Need
- Strong admin and CRM system skills (essential)
- High attention to detail and ability to manage multiple priorities
- Excellent customer service and communication skills
- Experience working in a fast-paced, outcome-focused environment
- Current First Aid/CPR Certificate
- NDIS Worker Screening Check, WWCC & Criminal Record Check
- Australian Driver’s Licence
Desirable (Not Essential)
- Experience in aged care, home care packages, or community care
- Understanding of HCP or Support at Home guidelines
- Registered or Enrolled Nurse with current AHPRA registration
