Support at Home Care Partner Monday–Friday | 8:30am–4:30pm | Permanent Fulltime Role Gympie

Duties and responsibilities include:

promoting and signing up new participants, explaining the Support at Home program.

Monitoring and following up Incidents and hazards, undertaking Assessments within the community.

completing care plans, home safety checklists, developing budgets, and providing ongoing case management to participants, demonstrating their goals, progressions and outcomes/achievements.

Provide financial and case management services to customers, and representatives where required.

Building professional relationships internally and externally and look for creative care solutions to achieve the best outcomes for clients

Ensure services are delivered are in line with the person centred and consumer directed principles and assist customers to access other alternative services where the services are no longer meeting their needs.

We support our participants in Budget management and liaise with ACAT, My Aged Care, PRODA and other regulatory bodies, GP’s, specialists, allied health to ensure the best outcomes possible.

There is also extensive daily administration tasks requiring attention to detail and follow up.

Ensure compliance with all Just Better Care policies and procedures, statutory and contractual requirements in relation to management of service delivery.

Apply and uphold principles of equity and anti-discrimination in the workplace and adhere to organisational and legislative Health, Safety and Environment requirements.
About you:

Caring, compassionate & friendly

Demonstrated high level verbal and written communication skills, including familiarity with, and confidence in using, the Microsoft Suite
-
Proven relationship management with all internal and external stakeholders, as well as good interpersonal, negotiation and problem solving skills
-
Ability to work as part of a team
- Previous customer service coordination experience in the health/aged care, non for profit, community services sector is desirable
What you will need:

NDIS Worker Screening Clearance / Criminal Record Check

Essential requirements include strong computer and administrative skills (CRM experience essential),
-
excellent multitasking, prioritisation and problem-solving abilities to excel in a fast paced working environment.

High-level customer service and communication skills.
-
Ability to work independently and as part of a team.
-
Experience working to KPI’s, billing for time.
-
Desirable – Experience working in community aged care and awareness of new Support at Home program.
