Care Partner (Aged Care)

Just Better Care Hawkesbury Nepean & Blue Mountains provide home and community care support services to Support at Home and NDIS Participants living in their own home. 

 

Care Partners perform an integral role in ensuring individualised services are developed and delivered to participants. Care Partners are responsible for the development, maintenance and review of individualised care plans in consultation with the participant, their family or guardians and the key health professionals.

 

This is an office based role at our office in Richmond NSW, and will involve the provision of: 

  • Undertaking comprehensive participant care planning by identifying and assessing participant needs, goals, preferences and existing supports
  • Conduct non-clinical participant assessments and complete documentation in accordance with Program Guidelines
  • Ensure efficient service coordination by communication and coordination with workers involved in the delivery of services, and with the participant and their family and informal carers
  • Monitor changing care needs and escalate new or emerging clinical risks to the Clinical Care Partner for review

Duties and responsibilities include:

  • Ensure that care management activity is delivered at least monthly to participants.
  • Ensure care plans are developed in partnership with the participant and their representative/s.
  • Ensure care plans are monitored and reviewed on a regular and ongoing basis (i.e. at least once every 12 months) and/or in response to changes in participant needs, services, circumstances and goals.
  • Reviewing agreements where participants care plans change.
  • Ensure services are delivered in line with the person centred and consumer directed principles.
  • Assist participants to access other alternative services where the services are no longer meeting their needs, including facilitating transitions in care where applicable.
  • Liaise with relevant community based agencies and service providers to ensure an integrated and cohesive approach to service delivery and to ensure standards are met in accordance with program objectives.
  • Monitor, review and evaluate ongoing care discussions, case conferencing with relevant team members and/or external service providers.
  • Ensuring that participants changing needs and emerging risks are monitored and reviewed.
  • Support participants to make informed decisions and integrating re-ablement approaches.
  • Demonstrating a commitment to best practice, ensuring that all services are provided in a caring and respectful way in accordance with Just Better Care’s policy and procedures, and legislative requirements.
  • Monitoring and communicating on the customer’s wellbeing and reporting on progress of customers.
  • Demonstrating a commitment to best practice and evidence of continued professional development.
  • Participating in staff meetings and attending training opportunities when required.

  • Proven relationship management with all internal and external stakeholders, as well as good interpersonal, negotiation and problem solving skills
  • Ability to work as part of a team
  • Experience in home and community care / case management

 

What you will need:

  • Relevant qualification and/or experience preferred e.g. Certificate IV Individual Support or Community Services or equivalent
  • NDIS Worker Screening Clearance
  • Current CPR / First Aid Certificate
  • Current Australian Driver’s Licence
  • Proof of Vaccination (where relevant) i.e. COVID-19 vaccination certificate
