About the role
The Package Manager role will perform an integral role in ensuring that our individualised services are developed and delivered to customers, through development, maintenance and review of individualised support plans in consultation with customers, their family or guardians and the key health professionals.
Working days: Monday to Friday 8:30am - 4:30pm.
Key Tasks:
Undertake comprehensive customer assessments and planning by assessing, monitoring, reassessing and reviewing the needs of the customer.
Ensure appropriate information is provided and explained to the customer at all times regarding service agreements etc.
Where applicable, develop strategies which will address the identified needs of the customer in consultation with their representative/s where appropriate, and connection to Allied Health Professionals in accordance with funding guidelines.
Provide financial and case management services to customers, and representatives where required
Uphold compliance to Policies & Procedures and WHS and the Just Better Care culture

Skills and experience
Essential:
Qualifications:
- EN Diploma (Endorsed Enrolled Nurse) - With current AHPRA registration
- Cert IV in Training & Assessment would be highly regarded
Ability to develop and maintain links and supportive networks with specialist services.
Previous experience in a Case Management role.
Ability to work flexibly to respond to changing customers' needs.
Ability to coordinate services according to relevant program guidelines.
Experience of working in a multi-disciplinary team.
Experience maintaining computerised customer information systems.
High level administrative and organisational skills.
High level of written and verbal communication skills.
Demonstrated ability to contribute positively to the team.
Current and clear Criminal Record Check/Blue Card/Yellow Card (or willingness to apply upon successful employment)
Current Drivers Licence.
Proof of of Vaccination (where relevant) i.e. COVID-19 vaccination certificate
Desired:
Previous customer service coordination experience in the health/aged care, non for profit, community services sector.
Experience as an EEN 5 years +
Experience/Knowledge of NDIS/Home Care Package legislative requirements.
Experience with NDIS/PRODA
