We are currently seeking a Customer Care Lead (Home Care Case Manager) to join our friendly and supportive Home and Community Care team. The successful candidate for this role will have a Metropolitan client base and travel around the Perth Metro area will be required to visit clients.  We have pool cars available at our Bentley office, or mileage can be paid for use of own vehicle.

Our Juniper Home and Community Care team provides community based care programs that assist our aged customers to continue to live independently in their own homes. Renowned for our commitment to the delivery of person centred care and best practice in relation to the provision of quality care, you will be working in a supportive environment that encourages initiative and leadership.

The Customer Care Lead operates within a multidisciplinary team structure and is the key contact and relationship champion with customers, their carers and significant others. The Customer Care Lead ensures collaboration and engagement, ensuring a person-centred and evidence-based approach to services are provided to all Home and Community Care team customers. In this role you will be accountable for the standard of care provided by Community Support Workers and for the management and coordination of a portfolio of government funded programs including Home Care Packages (HCP) and Commonwealth Home Support Program (CHSP), in accordance with Aged Care Quality Standards, Aged Care Act 1997 and relevant program guidelines. Care and services are to be within budget, responsive, timely and empowering, enabling the client to lead a continued safe and independent life within their own home.

Key Responsibilities of this role:

  • Management, coordination and service planning of community-based funding programs
  • People management through the employment life cycle, including recruitment, orientation, performance management, development, leave, and separations
  • Supervisory Responsibilities: Responsible for the supervision and support of community support workers. Acting as a role model while working positively with the team to ensure a cooperative approach to service delivery
  • Ensure compliance with Occupational, Safety and Health requirements including appropriate Injury Risk Management 

The successful candidate must meet these selection criteria:

  • Minimum Cert IV in Individual Support or equivalent
  • Knowledge of service provision, co-ordination, allocation and compliance with contractual obligations
  • Well-developed communication and interpersonal skills
  • Decision making, time management and problem-solving skills
  • A demonstrated empathetic and flexible approach
  • Computer literacy in word processing, spreadsheets, and calendar management
  • The ability to work autonomously with a team focus
  • A demonstrated understanding of health care within a community environment to co-ordinate quality service delivery
  • Experience in leading and expanding a multidisciplinary service across geographically dispersed locations within Not for Profit, Aged Care, Community or Health related industry
  • Experience in case management of government funded packages, community resources and relevant legislation in the provision of community and/or aged care services
  • Excellent people management experience and ability to confidently lead teams
  • Skills in coaching, developing, and engaging a large, geographically dispersed, and high performing team
  • Current WA driver’s licence and reliable vehicle, both essential
  • Received at least two covid-19 vaccinations and have had, or willing to obtain, influenza vaccination annually
