About the role:
We are currently seeking a Physiotherapist to join our friendly and supportive Home and Community Care team and our Transition Care Program Team in the North Metropolitan area of Perth. Our Juniper Home and Community Care team provides community and site based care programs that assist our aged customers to continue to live as independently as possible. Our Juniper Transition Care Program Team provides short-term care that seeks to optimise the functioning and independence of older adults after a hospital stay. Renowned for our commitment to the delivery of person centred care and best practice in relation to the provision of quality care, you will be working in a supportive environment that encourages initiative.
As a Juniper Physiotherapist you will use evidence-based interventions and a reablement approach to improve the independence of customers. This is an autonomous role that requires you to travel in the North Metropolitan area providing care to our customers in the community and at our TCP sites. The successful candidate will have a customer base in the North Metropolitan area and have access to pool cars or paid mileage, as well as a laptop and phone.
There is the option of joining our team on either a part-time or full-time basis. There is no shift work required, with work hours Monday to Friday during core business hours. To be considered for a part-time position you must be available at least three days each week. Shifts and hours will be negotiated with successful candidates and there is capacity for a partially hybrid working arrangement.
Key Responsibilities:
- Conduct comprehensive assessments, develop personalised care plans, and implement effective strategies to meet each client's unique needs
- Design both individual and group exercise programs to keep clients motivated and engaged to optimise their function and independence
- Through equipment assessment and prescription, identify and provide solutions that support client autonomy and well-being
- Foster collaboration and successful outcomes by communicating effectively with clients, families, and caregivers
- Ensure seamless care coordination and accurate progress tracking through meticulous record-keeping
- Work closely and collaboratively with the multidisciplinary team and Therapy Assistants to achieve the desired outcomes for your clients
Key requirements:
- Current FULL registration with AHPRA as a Physiotherapist
- Ability to relate to elderly customers with empathy whilst working effectively with other health professionals and service providers
- Excellent interpersonal and communication skills
- Current Australian driver’s licence and reliable vehicle - both essential
