Key Worker - Disability Support

We are seeking a key worker to lead a small team of a sociable outgoing 29 year old young man who has many roles and community involvements, but wants to build his independence, gain additional employment, contribute to his community, live an active and healthy lifestyle, and maintain and extend his wide circle of friendships.  

 

 The Keyworker would be responsible for:

  • Supporting the vision of a meaningful life and rich relationships by paying attention to both formal (paid) and informal (unpaid) supports
  • Nurture existing relationships and investigate a range of employment and other opportunities within his community
  • Oversee regular roster and work within budget to fill any gaps/new hours
  • Monitor messages and group chats around the young man’s support
  • Facilitate and lead a team through team meetings, mentoring, coaching and development
  • Be a bridge between the young person/his family and the support workers - ensuring the needs of the support workers are met whilst centering the young person/ the family's vision
  • Help the person to create a welcoming and functional home with his flatmate, and build networks in his neighbourhood
  • Work as part of a team, yet be able to work autonomously and on task 
  • Assist with Recruitment when required 

 

Selection Criteria:

  • Demonstrated ability to work alongside and advocate for people with a disability, and the capacity to encourage a shared vision around the young man
  • Demonstrated understanding of what it means to work in right relationship with people with disability, their families and committed others,
  • Demonstrated community building approaches in supporting people at risk of marginalisation to pursue a meaningful inclusive life,
  • Excellent interpersonal, verbal and written communication skills,
  • Demonstrated ability to work both independently as well as to inspire, motivate, coach and supervise a team of support workers
  • Ability to think creatively and developmentally to support a young man to live, work and contribute to his community 

 

Jobseeker Requirements:

  • Value and respect that you work directly for the young man and their family
  • Criminal History Check e.g. NDIS workers Screening (or willing to obtain)
  • Availability for between 5-8 hours per week during weekdays, with flexibility to respond to calls at agreed times and also flexibility for evening meetings (once per 6-8 weeks)
  • Reasonable location to Fairfield area – local knowledge of inner southside and proximity will be well regarded.
