About the Role:

As the Clinical Case Manager Lead, you'll step into a pivotal leadership position within our care team. Your primary responsibility will be to oversee the clinical and administrative aspects of Home Care Packages at The Landings. This entails ensuring that our clinical case management and reporting processes are meticulously maintained, while also spearheading the integration and coordination of resident care and support services. Your leadership will be instrumental in upholding our commitment to delivering top-tier care and enhancing the overall well-being of our residents.

 

Key Responsibilities:

  • Lead and manage a team of Clinical Case Managers, providing guidance and support to ensure optimal performance and adherence to protocols.
  • Oversee the clinical and administrative aspects of Home Care Packages, ensuring compliance with regulatory standards and best practices.
  • Maintain accurate and up-to-date clinical case management records and reports.
  • Collaborate with internal stakeholders to ensure seamless integration and coordination of resident care and support services.
  • Identify opportunities for process improvement and implement strategies to enhance efficiency and effectiveness.
  • Foster a culture of excellence, teamwork, and continuous learning within the clinical case management team.

 

Requirements:

  • Bachelor's degree in Nursing, Social Work, or a related field; Master's degree preferred.
  • Experience in clinical case management, preferably in a home care or aged care setting.
  • Proven leadership experience, with the ability to effectively manage and inspire a team.
  • Strong understanding of regulatory requirements and best practices related to clinical case management.
  • Excellent communication and interpersonal skills, with the ability to collaborate effectively with internal and external stakeholders.
  • Demonstrated problem-solving skills and a proactive approach to identifying and addressing challenges.
  • Commitment to upholding ethical standards and providing compassionate care to our residents.

  • A sign-on bonus applies to this role. The successful applicant will receive a $1500 sign-on bonus, payable in three stages across 18 months from commencement, specifically $500 payable at six, twelve and 18 months of service. As our sign-on bonus is intended to encourage successful applicants to stay at LDK, you will only receive the full bonus if you stay with us for 18 months.
