About the Vacancy

Are you a Registered Nurse with experience in a home care or residential aged care environment, ready to embrace a transformative opportunity in seniors’ care? LDK are seeking an experienced clinician and skilled communicator, able to bring together a variety of stakeholders to achieve beautiful outcomes for senior Australians.

LDK’s newest development Amberfield Village has stimulated considerable community buzz in Canberra, with enthusiastic new buyers optimistically planning for their ageing journey in this 5-star environment.  This is a unique opportunity to join the pre-opening team of Amberfield Village, as you prepare to undertake clinical assessment and care planning for a cohort of people who can’t wait to move in!

The Clinical Case Manager is a pivotal, high profile and outwardly facing role in Amberfield Village, where you will be required to effectively liaise with external health service providers, internal teams (sales/resident services/lifestyle/operations), potential residents and their families, and our own clinical care team to ensure a smooth move in experience for all residents requiring care. 

 

You will be familiar with government home care funding, and have current AHPRA registration as a Registered Nurse, including recent clinical experience. Most of all you will be ready to leave your preconceptions about aged care at the door.

About You

  • Current registration with AHPRA as a Registered Nurse.
  • Member of a Professional Organisation – Desirable.
  • Full working rights in Australia.
  • Alignment to a values-based organisation.

Professional Experience

  • Experience working in aged care as a Registered Nurse, home or community care environments would be highly desirable.
  • Recent clinical experience relevant to the position, including demonstrated ability to liaise with key stakeholders to achieve optimum nursing and care outcomes.
