About the Role

Reporting to the Clinical Case Manager Lead, the Clinical Case Manager supports integrated and coordinated resident care and service requirements, including pre-entry clinical assessment, move-in support, care planning, care delivery, care evaluation, referral and accessing externally funded care (with associated documentation), coordination of support for residents and networking. 

The Clinical Case Manager has an important leadership role within the care team and liaises with a wide range of internal and external stakeholders.

  • Streamlining care for your portfolio of residents

  • Conducting timely and comprehensive clinical assessments

  • Overseeing resident support to determine precise care needs

  • Facilitating smooth transitions to LDK Homecare AP where needed

  • Welcoming new residents to the Village, introducing them to the care team and assistive technologies

  • Assisting residents in accessing externally funded care

  • Coordinating aged care assessments through ACAT/MyAgedCare

  • Navigating Commonwealth Home Care Packages and Support Programs

  • Handling Veterans' care coordination, including GP communication and documentation

  • Assessing eligibility for external care services

  • Collaborating with the Clinical Case Manager Lead to meet resident goals and preferences

Qualifications/Certificates

  • AHPRA registration (Registered Nurse)

Professional Experience

  • Experience in the community care sector/with home care packages
  • Aged care nursing experience desirable
  • Experience working as a member of a multidisciplinary team
