Your new role

We are currently seeking a motivated and enthusiastic professional, with a commitment to social justice, to join our multi-disciplinary community team at our Lyndell White House in Coburg as a Housing Support Worker within the Housing Support for the Aged Program on a Full Time basis. This program is funded by the Victorian Department of Health as part of the Low-Cost Accommodation Support Programs and has been developed to support the client population to maintain stable long-term housing arrangements and improve their health and well-being.

Please note you must have your own suitable car and valid driver’s licence to be considered for this role.

 

Your duties and responsibilities will include:

  • Engage and establish a relationship of trust with clients.
  • Assist the Manager and Housing Staff prioritise and decide who is nominated into any vacant properties.
  • Identify and conduct ongoing client needs assessments on the unmet care and support needs of clients, taking into account their circumstances and preferences.
  • Assist Clients where necessary, to move into accommodation with Wintringham or Housing Choices Australia, including co-ordinating relocation tasks as required.
  • Regularly visit and monitor the health and welfare of clients.
  • Provide practical and immediate assistance to clients to overcome health or other crises.
  • Develop, implement, and review Case Files and assessments for each client.
  • Co-ordinate the delivery of service to clients and advocate on their behalf if required.
  • Monitor the degree to which client needs are being met by the program and other service providers.
  • Develop and implement range of social support strategies and provide practical assistance to link clients into services that meet their social support needs.
  • Provide case management to co-ordinate client access to services.
  • Provide practical and flexible assistance such as making and helping clients to get to appointments and social support.
  • Assist in enhancing responsiveness of local service providers to the needs of the target group and the achievement of sustainable improvements in the quality of service delivery.

 

 

 

We are looking for candidates with the following:

  • An approved tertiary qualification in Social Work or equivalent Allied Health qualification.
  • First Aid Certificate Level 2. (Mandatory)
  • Experience working with older, homeless people and people with unmet, complex care needs.
  • Outreach skills including the ability to visit and establish rapport with both socially isolated people and people with challenging behaviours.
  • Current vehicle (Preferable)
  • Current drivers licence (Mandatory)
