About the role
As our Support Coordinator, you will work closely with customers to understand their NDIS funded supports, engage with appropriate service providers, and monitor progress toward the achievement of their personal goals. An important function of Support Coordination is to assist customers to identify community or mainstream supports that can also assist them to achieve their personal goals. This is a fixed term role for 12 months.
Role requirements may include, but not limited to, the following:
- Work closely with customers to identify and engage the range of services necessary for them to meet their goals.
- Demonstrate extensive consideration of and access to services, outside of Multicap.
- Provide support to individuals, family members and other stakeholders to ensure that planning processes are fully consultative and responsive to views and aspirations of key decision makers.
- Develop and maintain mutually beneficial working relationships with staff, internal groups and external bodies to establish viable interactive partnerships.
- Maintain effective team practices, through appropriate communication channels both with members of the team and with all other Multicap staff members.
- Develop productive partnerships with external service providers, and mainstream agencies in the delivery of a high quality outcome for Support Coordination customers and families.
- Support the work of the organisation to develop tools and processes that effectively evaluate service activities and outcomes.
- Provide advice guidance and support to staff teams to ensure that individuals, particularly those with complex needs have plans that are effective, current and reviewed appropriately.
- Support the work of the organisation through internal monitoring and audit processes to develop and maintain on-going best practice and ensure that the organisation maintains external accreditation as required.
- Provide reports as required and where funded to both the NDIA and customer.
- Deliver services to customers which effectively, responsibly and transparently, utilises personal funding.
- Meet individual performance KPI’s regarding revenue generation with particular regard to weekly and monthly billed hours.
The successful applicant will:
- Hold qualifications in Human Services or Social Sciences, or equivalent (desirable but not essential)
- Possess and demonstrate sound disability (or allied health/welfare/education) experience, knowledge, and/or training relevant to the position.
- Have experience in coordinating the provision of person centred supports to people with a disability and their families.
- Have experience in the delivery of contemporary human services practices in relation to people with challenging behaviours and complex support needs.
- Have knowledge of the State Disability Services Act and the 2014 Amendment relating to restrictive practices.
- Advanced understanding of positive behaviour support frameworks.
- High level interpersonal skills and experience supporting people in situations of high emotions.
- Demonstrate a high level of interpersonal and communicative skills and ability to develop strong relationships with internal and external stakeholders.
- Demonstrate a high level of written and verbal communication skills.
- Demonstrate problem solving skills to successfully identify problems, develop solutions and implement these using a logical and systematic approach.
Mandatory requirement:
Please note that the successful applicant will be required to hold or be willing to obtain the following:
- Hold a current QLD Drivers Licence
- Hold a current blue (working with children) and yellow (positive notice card) or be willing to obtain one
- Hold a current Senior First Aid and CPR certificate
- Be willing to undertake and pass a criminal history clearance
Salary sacrifice + Meal entertainment
Attractive Salary packaging including $15,900 packaging limit per FBT year (1 April- 31 March) and $2,550 per year for meals and entertainment expenses.
