About the role
We have an exciting part-time temporary opportunity for an Age Well Planner to join the team. Be a part of the development and growth of the Merri-bek Navigation Service. The Age Well Planner is responsible for supporting Merri-bek residents to be able to access the services they need, by increasing their knowledge of My Aged Care, developing an Age Well Guide, providing regular "Ask us Anything" sessions in the community as well as one on one support via the phone or In person and at our local events and festivals. As well as regular evaluation and reporting of progress.
Skills and experience required to perform this role
- A degree or diploma in an allied health or relevant profession (e.g. health promotion, community development or social sciences) or equivalent mix of lesser formal qualifications and substantial experience working with diverse older people or people requiring additional support
- Experience working in the health or community services sector
- Sound knowledge of aged care service system, social determinates of health in older people, policy directions with the aged care sector and ageing well principles
- An applicant with lived experience of the aged care system or a self-identified older person
- Familiarity with the Merri-bek community and service organisations
- Ability to speak a language other than English that is relevant to the older Merri-bek community (such as Italian, Greek or Arabic)
