Who are we?
Wintringham is a specialist not-for-profit organisation providing an extensive range of aged care, housing and related support services to men and women aged 50-plus who have been homeless or at risk of homelessness. We employ over 1000 staff and operate from multiple offices across Melbourne and regional Victoria. Our Head Office provides the strategic direction and support services for Wintringham’s residential, housing and community care services.
Our friendly team of NDIS professionals are committed to ensuring that clients who meet NDIS eligibility, achieve access to the Scheme and are provided with the highest standard of NDIS services and options available. Some of your clients will live in the community and others in residential aged care settings. Most have no family or support people to assist them to manage their services which is why our role is so rewarding.
Your new role
An opportunity exists for a proactive, enthusiastic and experienced NDIS Support Coordinator wanting to use their skills to work with prematurely ageing clients experiencing homelessness and disadvantage. This multi-faceted role will be perfect for you if you have a Case Management, Allied Health or Disability Services background and tertiary qualification. The position will run for a year from the 24th of February 2025 to the 24th of February 2026. We also offer a highly flexible opportunity allowing you to work from Wintringham office(s) closest to you as well as remotely when you’re not travelling with/between clients.
Coordination of Supports includes:
- Understanding and implementing NDIS Plans
- Connecting with supports and services
- Designing support approaches
- Establishing supports and networks of providers
- Targeted Support Coordination
- Written reports to the NDIA and preparation of annual reviews/assessments
Other duties and responsibilities and duties include:
- Advocating and responding to clients’ needs and identifying appropriate solutions
- Working closely with NDIA Local Area Coordinators, NDIA Planners, Case Managers, Residential Aged Care Facilities and Wintringham’s various specialist support programs including housing and outreach, home care packages, recreation and rostering.
- Linking clients with a variety of social, emotional, recreational and allied services based on their assessed disability needs outlined within the NDIS Plan
- Effectively manage services provided within budgetary constraints
We are looking for candidates with the following:
- An approved tertiary qualification in Social Work, Nursing, Allied Health or Community Services
- Experience and understanding of the role of the NDIS Support Coordinator
- Current drivers licence
- Your own reliable vehicle
- Flexibility and a willingness to travel
- Excellent written and verbal communication skills
- An understanding of duty of care and privacy and the ability to apply these in day-to-day practice.

Applicants who have lived in a country other than Australia for a period longer than 12 months in the last 10 years may be required to undergo an International Police Check.
Successful candidates will be required to comply with the Wintringham Health and Immunisation policy including having three doses of a COVID-19 vaccine before commencing employment and must provide evidence.
