About the Facility

Grandton Aged Care is a boutique 26‑bed residential aged care service within a 14‑storey vertical community in Applecross, overlooking the Swan River. Residents enjoy premium accommodation with access to amenities such as an indoor pool, cinema, gym and the Lotus by Roshana restaurant.
We are looking for a Registered Nurse to join our Home Care team at Grandton Aged Care, in a (casual or permanent part‑time) capacity.

About the Role

We are seeking an experienced Registered Nurse to join our 26‑suite Aged Care Facility, which also provides Private and Home Care Services within the independent apartments at Grandton Applecross. The RN is responsible for delivering the highest quality, person‑centred care and ensuring clinical excellence across all services, consistently exceeding industry standards.

This role is supported by the facility management team, allied health professionals, and administration staff, with additional leadership and guidance from our Head Office in Burswood.

 

Duties and Responsibilities:

  • Provides leadership to Care Partners, ensuring care needs of the resident are met in accordance with clinical needs and the residents’ care plans.
  • In conjunction with the Manager Residential Care and Clinical Manager, ensures all assessments are completed for each resident during the admission process, then as required as residents’ care needs change.
  • Develops care plans for each resident, utilising information from assessments and in liaison with resident, family, GP, allied health professional, and other staff.
  • Informs relevant managers of any exceptional events, including changes in resident status, in a timely manner.
  • Responds in an efficient and professional manner to any quality issues and actively contributes to the continuous improvement system.
  • Promotes and supports consistency for individual residents when assigning care, delegating activities, and providing direction to Enrolled Nurses, Care Partners, and others.
  • Models’ enablement of residents whilst attending to resident needs and providing direction and assistance to other staff members in performing their duties.
  • Actively builds trusting relationships with individual residents that are based on dignity and respect to optimise cooperation, communicating in a manner that is appropriate to each individual’s strengths and limitations.
  • Implements and oversees, safe working practices to comply with Work Health and Safety, Food Safety and other relevant legislation and policy, including reporting and investigation of identified hazards and incidents.

 

Skills, Attributes and Experience:

  • Tertiary qualifications in Nursing and current registration with AHPRA as a Registered Nurse.
  • Proven high level of clinical competencies with excellent clinical analysis, interpretation, and planning skills.
  • Must hold or be willing to obtain a National Police Clearance.
  • SIRS management - incident reporting and investigation.
  • Experience in aged care working in a residential/community care setting.
  • Able to lead and influence others.
  • Relates to the aged care population with empathy, understanding and respect.
  • Sound understanding of care assessment and provision within the Aged and Health Care sectors.
  • Professional written and verbal communication skills.
  • Ability to work flexibly within a multidisciplinary team environment.
