About the Role

Working closely with the multidisciplinary team and other stakeholders within the Grandton community, this position is responsible for coordinating person-centered leisure and lifestyle support to residents. As team leader, this position plans and leads the implementation, monitoring and evaluation of individualized and innovative programs, to promote the physical and emotional wellbeing of residents through involvement in the meaningful activities of their life.

The Lifestyle Coordinator has responsibility for recruitment, induction and supervision of volunteers.

Duties and Responsibilities

  • Compiles a life history profile for each resident including social, emotional, cultural and spiritual preferences and needs, personal interests and preferences.
  • Facilitates dissemination of life history information and personal preferences to the multidisciplinary team.
  • In collaboration with care teams, residents and families, develops an individualised Lifestyle Plan with each resident and/or their representative that integrates into their care plan.
  • Works with the therapy team to prepare and develop weekly programs suitable for individual and collective residents' needs, incorporating the celebration of culturally and socially significant events.
  • Assigns responsibility for therapy and leisure program implementation to rostered Therapy and Leisure Partners.
  • Supports Therapy and Leisure Partners to adapt activities to residents' needs as appropriate and required, liaising with and taking direction from therapists as needed.
  • Follows residents' care plans in the provision of person-centered support during leisure, lifestyle and therapeutic activity.
  • Optimizes resident cooperation by actively building trusting relationships with individuals that are based on dignity and respect.
  • Ensures ongoing communication with the Occupational Therapist and Physiotherapist regarding program outcomes.

 

Skills, Attributes and Experience

Essential:

  • Minimum 3 years’ experience as a Lifestyle Coordinator.
  • Completion of Certificate IV Lifestyle and Leisure or experience in such a role.
  • Must have a current First Aid Certificate, including CPR.
  • Must hold a Current National Police Clearance.
  • Demonstrated ability to plan, implement and evaluate innovative programs related to care/services.
  • Experience in leading and delivering Leisure & Lifestyle within an Aged Care setting.
  • Demonstrated experience in using Microsoft Office and IT systems.
  • Demonstrated effective verbal and written communication skills.
  • Demonstrated understanding of person-centred principles and the ability to engage people in meaningful activity.
  • Knowledge of Aged Care standards and Compliance with relevant state government health mandates.
  • Professional written and verbal communication skills.
  • Demonstrated understanding of the needs of people with dementia in relation to leisure and therapeutic activity.
  • Ability to complete busy workload and respond to changing priorities.
