About the Role

The Purpose of the role of the Home Care Coordinator is to provide support to our Home Care clients by implementing and delivering personalized service plans that are tailored to the client's goals and allocated budget. Additionally, the coordinator is responsible for identifying support workers for the clients, scheduling their work, and supervising them to ensure the effective delivery of our service promises.

Key Responsibilities:

  • Monthly Audit Calendars and Reporting to Management
    • Following a monthly audit calendar to ensure timely completion of audit tasks.
    • Providing comprehensive and accurate reports to management based on the audit findings.
  • Continuous Improvement Processes and Practices
    • Emphasizing continuous improvement in work processes and practices.
    • Regularly reviewing and evaluating existing processes to identify areas for enhancement and optimization.
  • Scheduling, Supervising, Performance Managing, and Submitting Timesheets for Support Workers
    • Scheduling and supervising support workers' tasks and activities.
    • Managing the performance of support workers and ensuring the timely submission of their timesheets.
  • Monitoring and Reviewing Client Support and Wellbeing
    • Reviewing client goals to align services with their objectives.
    • Monitoring and measuring individual progress towards set goals.
    • Documenting outcomes for review with client stakeholders to ensure transparency and accountability.

  • Performing other duties as directed.

Skills, Attributes, and Experience

We are seeking an ideal candidate with prior experience in a community aged care setting and familiarity with the provision of home care packages. The candidate should have a proven track record of achieving client satisfaction by connecting clients to support and providing engaging services.

Essential

  • Previous experience in a coordination capacity for a community aged care organization.
  • The ability to work autonomously, self-motivate, and operate with minimal supervision is necessary for success in the role.
  • Excellent communication and interpersonal skills, including customer service. Proficient in utilizing various IT systems, including the Microsoft suite encompassing Word, PowerPoint, Publisher, and Outlook (not limited to).
  • High levels of organizational, time management, and priority-setting skills.
  • Must possess or be willing to obtain a police clearance within 12 months.
  • Must possess full Australian working rights, a valid license, and a reliable vehicle.
