About the Role

Working closely with the multidisciplinary team and other stakeholders within the facility, this position is responsible for the provision of person-centered, individual care to residents within the Grandton community. 

The function of this position is to promote the physical and emotional wellbeing of residents, by partnering with residents to be involved in the meaningful activities of their life. This includes everyday activities such as eating, personal hygiene tasks (such as bathing, dressing, going to the toilet), moving around, household tasks and leisure activities. Care Partners coordinate with other staff to achieve these outcomes and liaise directly with nursing staff regarding any specific health concerns.

Duties and Responsibilities:

  • Follows care plan instructions to assist residents to maintain personal hygiene, nutritional status and mobility in a manner that maintains the safety of the resident and staff. 
  • Supports residents to use and develop their abilities to engage in any activity that is personally meaningful to residents including everyday activities.
  • Uses flexible and creative approaches with residents to meet duty of care whilst supporting residents’ rights to exercise maximum choice and control during everyday activities and respecting residents’ rights to decline offers of support. 
  • Recognises resident preferences, including personally significant cultural and spiritual practices, and incorporates these into everyday activities. 
  • Actively supports residents to build mutually beneficial relationships with other residents for the purpose of companionship.
  • Assists with preparation of beverages and plating and/or serving of food as required, including residents as able.
  • Engages cooperatively with residents to assist them to take packaged medication in accordance with medication policy and practice, and as prescribed by the General Practitioner.
  • Maintains a clean, comfortable, and homely environment in partnership with residents as able.

Required Skills, Attributes and Experience

  • Minimum 3 years’ experience in Aged Care.
  • Minimum Certificate III in Individual Support (previously known as Certificate III in Aged Care).
  • Must hold or be willing to obtain a First Aid Certificate, including CPR.
  • Must hold or be willing to obtain a National Police Clearance.
  • Must have or be willing to obtain Food Safety training and Manual Handling training.
  • Medication competency.
  • Ability to engage residents though a person-centered approach to care and support.
  • Demonstrated ability to work independently and report as required to supervisor, as well as works well and contributes effectively within a team environment.
  • Able to communicate well and build relationships with people from many walks of life including those from Culturally and Linguistically Diverse (CALD) backgrounds.
  • Excellent communication skills and articulate in English language.
  • Positive and friendly personality with a passion for assisting our residents to maintain their quality of life. 
  • Basic computer skills.
