About the Role
The Home Care Support Worker provides support to customers, specifically in the areas of identified goals as per the Care Plan. Working closely with the Home Care team and other key stakeholders this position is responsible for the provision of person-centred care to our Home Care Customers.
The function of this position is to promote the physical, emotional and spiritual wellbeing of home care customers and being involved in their care whilst at home. Promoting their independence and supporting them to live safely in the community is essential. Services to be provided to support Consumer include personal care, domestic assistance, meal preparation, medication prompts, transport to and from the shops as well as medical appointments, social support and companionship.
Duties and Responsibilities

Home Care Support Work
- Provide services listed on the customers care plan such as domestic assistance, personal care, meals and shopping, transport to medical appointments, social support.
- Recognises Consumer preferences including personally significant cultural and spiritual practices and incorporates these into everyday activities.
- Assists with preparation of customers meals as required as per care plan.
- Provide transport to medical appointments as per care plan.
- Undertake Laundry tasks and cleaning as required.
Customers, Families and Services
Work with customers and families in a manner that will:
- Promote independence to enable them to live at home for as long as possible.
- Promote holistic services that focus on total wellbeing, including physical health.
Leadership and Management
- Support the Manager.
- Support strategic objectives through the delivery of comprehensive services in provision of Home Care Services.
- Assist the Manager to resolve any Consumer complaints where required and implement the learning.
- Contribute to the constant improvement and risk management of Roshana's Home Care services.
- Report any critical incidents or notifiable incidents to your manager.
- Provide input into development of programs as required.
- Promote Roshana Group as an employer of choice within a safe working environment.
- Participate and contribute to a Consumer service continuous improvement plan.
Documentation
- Provide Consumer progress notes in the Nightingale System.
- Record information onto relevant forms such as an incident report form.
Occupational Health & Safety
- Ensure the work environment is safe and healthy and complies with all OSH requirements.
- Take a shared responsibility to ensure the safety and well-being of themselves and others within the workplace.
- Work in a safe manner while exercising due care and caution.
- Report any unsafe practices or equipment.
- Wear/use all protective equipment provided and as instructed.
- Address breaches of OS&H policies with the Home Care Manager.
All Staff are to be aware of and work in a manner that is compliant with:
- Age Care Act
- Aged Care Charter of Rights.
- Work Health and Safety legislation.
- All Roshana Group's policies and practices
Skills, Attributes and Experience
- Minimum 3 years’ experience in Aged Care.
- Minimum Certificate III in Aged Care or higher.
- Must have or be willing to obtain a First Aid Certificate, including CPR.
- Must have or be willing to complete Food Safety training and Manual Handling training
- Medication competency.
- Must hold a Current National Police Clearance.
- Ability to develop ethical work practices including an understanding of appropriate personal boundaries.
- Ability to work within a multidisciplinary team environment, promoting wellbeing and holistic health
- services.
- Demonstrated high level written and verbal communication skills and articulate in English language.
- Demonstrated understanding of the person-centered approach.
