We are excited to offer a unique dual Registered Nurse (RN) opportunity, supporting both aged care residents and home care clients at our Grandton facility in Applecross. This role will involve travel to visit home care clients outside the facility as needed.

About the Role

As a Registered Nurse, you will work closely with residents, home care clients, their families, and a multidisciplinary team to assess individual strengths and needs and develop person-centred support plans that promote physical and emotional wellbeing. This dual role covers both direct clinical care within the facility and visits to home care clients in the community

 

Key Responsibilities

  • Develop and maintain care plans, liaising with residents, clients, families, GPs, allied health professionals, and other staff.
  • Communicate any exceptional events or changes in client/resident status to relevant managers promptly.
  • Respond professionally to quality issues and contribute actively to continuous improvement.
  • Delegate, assign, and provide direction to Enrolled Nurses, Care Partners, and other staff, supporting consistency and quality in care.
  • Build trusting relationships based on dignity and respect, adapting communication to individual strengths and needs.
  • Implement and oversee safe working practices in compliance with Work Health and Safety, Food Safety, and other relevant legislation and policies.
  • Travel to provide care and support to home care clients in the community as required

 

Skills, Attributes, and Experience

  • Tertiary qualifications in Nursing and current AHPRA registration as a Registered Nurse.
  • Demonstrated high level of clinical competence, with strong clinical analysis, interpretation, and planning skills.
  • Must hold or be willing to obtain a National Police Clearance.
  • Experience with SIRS management, incident reporting, and investigation.
  • Experience in providing clinical care, both residential and home care settings.
  • Leadership skills and the ability to influence others.
  • Empathy, understanding, and respect for the aged care population.
  • Sound knowledge of care assessment and provision within the aged and health care sectors.
  • Excellent written and verbal communication skills.
  • Flexibility and ability to work collaboratively in a multidisciplinary team.
  • Willingness and ability to travel to home care clients as part of the role
