About The Opportunity

As a Mental Health Registered Nurse, you will play a crucial role in delivering high-quality care within a multidisciplinary team. Your primary responsibilities will include:

• Conducting clinical assessments and screenings to support individuals and groups
• Facilitating access to health checks and healthy lifestyle programs to promote overall community health.
• Collaborating effectively with internal and external service providers to ensure coordinated care for high-risk populations.
• Participating in health promotion activities in collaboration with key stakeholders.
• Engaging in continuous professional development to enhance clinical practice and knowledge.

About You

The successful candidate will have:

• A relevant nursing qualification and current registration with the AHPRA
• 3-5 years’ experience in clinical or community settings, with a focus on care coordination or case management.
• Experience working with patients experiencing mental illness and associated comorbidities.
• Strong communication and interpersonal skills, with the ability to engage with diverse stakeholders.
• Proficiency in health IT systems and communication technologies.
• Current registration to Work with Vulnerable People
• Current Driver’s Licence
