About The Opportunity

As the Clinical Care Coordinator, you will play a pivotal role in overseeing and delivering high-quality nursing care to residents.

Your responsibilities will include:

• Provide and monitor multi-disciplinary nursing care to all residents
• Support and promote the facility's care philosophy
• Communicate effectively with staff and external agencies
• Maintain knowledge of mental health conditions and workplace health & safety practices
• Lead and mentor nursing and care staff
• Ensure timely delivery of resident care and medication administration

About You

The successful candidate will have:

• Previous experience in Geriatric Nursing and Mental Health 
• NDIS knowledge (desirable)
• Current registration as a Registered Nurse with AHPRA
• Positive notice NDIS Worker Clearance
• Knowledge of AN-ACC funding guidelines
• Sound knowledge of industry-relevant Workplace Health & Safety and Infection Control practices

You will also have:

• Strong communication and interpersonal skills
• A proactive approach to staff development
• Excellent documentation skills
• A commitment to resident-centered care
