About The Company

Their services are delivered by a highly trained, professional and caring team, working collaboratively with other health professionals to ensure the right care at the right time.
About The Opportunity
The primary purpose of this position is to coordinate services for Commonwealth Home Support Programme (CHSP) clients, from initial intake and onboarding through to ongoing service delivery. This is a practical role that brings together client service coordination, administration and communication. - Managing client intake and onboarding, including preparing and completing sign-up documentation
- Clearly explaining CHSP and Support at Home services to clients and their families
- Coordinating new services and service changes with the rostering team
- Communicating service requirements to care workers
- Monitoring changing client needs, supporting service reviews and assisting with referrals for reassessment
- Maintaining accurate client records, service notes and compliance documentation
- Assisting with reporting, quality activities and general administration
About You
The successful candidate will have:
- Experience in aged care or community services, ideally within CHSP, Home Care Packages or Support at Home
- An understanding of service coordination, client onboarding and basic scheduling

- Confidence explaining aged care programs and services to clients and families
- Proficiency with client management systems and Microsoft Office
- A valid driver’s licence, full Australian work rights and a National Police Check (or willingness to obtain one)
It would also be desirable if you have:

- Familiarity with My Aged Care, CHSP guidelines and Support at Home
- Previous experience as a Coordinator or Intake Officer, with strong administration skills
