About the Position
Anglicare Southern Queensland has recently integrated our homelessness services for Women & Families across South-East Queensland and is located at Toowong, creating exciting new opportunities for human services professionals to make a significant contribution to our program and the outcomes we achieve in the community.

Working 60 hours per fortnight, the Intake and Assessment Case Worker, works as part of an integrated Case Management Team in providing accommodation, support and case management to women who are homeless or at risk of homelessness, young women who are pregnant or parenting, women with an intellectual disability, and families where one or both of the parents have an intellectual disability and are homeless or at risk of homelessness.
Working under the leadership of the Service Coordinator, you will undertake referral, assessment and intake processes for service users entering our accommodation, disability and targeted family support programs. This process will require utilisation of the Queensland Homelessness Information Platform for monitoring and entry purposes, a range of assessment tools, Procura database and SHIP database entry.
The Intake & Assessment Case Worker will utilise a strengths based approach, and have an understanding of:
- Trauma Informed Practice
- Recovery Oriented Practice
- Housing First Principles
Together with qualifications in Human Services field and/or relevant work experience within the field of adult and youth homelessness, intellectual disability and early intervention for targeted family support. The successful applicant will have demonstrated intake and assessment and case management experience
You will also require a Blue Card for Child Related Employment and a Yellow Disability Services exemption card and hold a current unrestricted Drivers’ Licence
