About the Position
Our West Moreton Community Services team are key in providing care and support to a range of clients, including Aged Care and Disability services. We work with people to explore new options, open doors and create solutions that enable people to live well in their own homes. Due to the upcoming implementation of NDIS, we are currently looking for a Disability Services Coordinator to join us on a full time basis.
This role’s overall purpose is to lead and coordinate the day-to-day operations of Disability Services, including assisting in the facilitation and monitoring of individualised support plans for clients; providing supervision to a team of staff; and providing leadership and support in practice delivery and governance.
The Disability Services Coordinator will also lead, develop and promote the service in the community, build partnerships with relevant stakeholders and undertake and participate in role-related project work as required. High level contribution to Continuous Quality Improvement (including Accreditation compliance) is an important element of the role, as is staff training and development.
About You
With a broad and well developed career within the Disability Services sector, you will already have significant exposure to, and understanding of NDIS and be able to offer:
- Demonstrated understanding of funding arrangements, access criteria and operational processes including governing legislation
- Experience working within a person-centred framework with a belief that each person has gifts, strengths and value to the community
- Management and coordination of activities to support clients including development of Individual Support Plans
- The ability to provide supportive leadership and mentorship to a team of staff and volunteers
- Highest level communication skills and the confidence to build effective relationships with clients, their families and other stakeholders
- Proven skills in crisis response and emergency decision making, supported by attention to detail and professionalism
- Excellent administrative skills which include record-keeping, assessment planning and general correspondence
- Knowledge of the HSQF and CQI
- Experience in providing staff development and training activities
Our successful candidate will also maintain the following qualifications:
- A three-year degree in Social Sciences, Human Services or equivalent practical experience
- A current National Police Certificate
- Valid Blue Card (Working with Children) and Yellow Card (Positive Notice - Disabilities)
- Current Queensland Drivers Licence and your own reliable vehicle
