Your new role
The Senior Care Partner is responsible for providing practice leadership, supervision and oversight of care management across Home Support services within the Barwon region. The role leads and supports Care Partners and Support Partners to deliver high quality, participant centred services that promote independence, dignity and wellbeing. Reporting to the Regional Manager Home Support (NW Metro, Barwon) this is a full-time permanent role, located in Geelong.
We are looking for candidates with the following
- Relevant qualifications in Nursing, Social Work, Welfare, Allied Health or another related discipline. A valid Victorian drivers licence
- Demonstrated experience in care management, assessment and service coordination within aged care, health, community services or a related sector, including supporting participants with complex needs.
- Proven leadership capability, with the ability to coach, supervise and develop staff, fostering collaboration, accountability and high-quality service delivery.
- Strong knowledge of aged care programs, funding models and regulatory requirements, including the Aged Care Act, Support at Home program, quality standards and associated compliance obligations.
- Demonstrated experience supporting participants with complex needs, including care planning, risk assessment, multidisciplinary coordination, referral pathways and service navigation.
- Experience managing budgets, caseloads and service utilisation, with the ability to support staff in achieving participant outcomes while maintaining financial accountability and program sustainability.
